SG01- Incident with No or Incorrect "Resulting From"

Error Colour Code:

Error Brief Description: Error Populates if “Resulting From" is left blank or anything selected other than ‘Safeguarding Concern’
Location: Safeguarding Adults > SA incidents >Maintain SA incident > Details

How to fix this error?

 

Go to Safeguarding Adults tab in the purple side bar menu in AIS. Click on SA Incidents tab. 

 

 

 

Click on the hyperlink on the reported date

 

In Maintain SA Incident screen, go to Incident Details section; select the appropriate contact with Safeguarding Concern from the drop- down list.

 

 

 

Click on “Save” icon

SG02 - Incident ended with no correct "Status"

 

Error Colour Code:

Error Brief Description:  Error populates if an incident has ended but a status of “Alleged-Pending Enquiry or "Suspected -Pending Enquiry" has remained unchanged. Need to be changed to one of the other options from the drop down list.

                      
Location: Safeguarding Adults > SA Incidents >Maintain SA incident > Details

 

How to fix this error?

 

Go to Safeguarding Adult tab in the purple side bar menu in AIS. Click on SA incidents tab. 

 

 

 

Click on the hyperlink on the reported date

 

In “Maintain SA incident screen, scroll down to “Victim Details” section and  choose the relevant Status from the drop-down list

SG03 - Missing Incident Locations Section

Error Colour Code:

Error Brief Description: Incident Location Section is mandatory. If left blank, then error.                                                                

 Location: Safeguarding Adults > SA incidents> Appropriate reported date >Maintain SA incidents>

How to fix this error?

 

Go to Safeguarding Adult tab in the purple side bar menu in AIS. Click on SA incidents tab. 

 

 

Select the appropriate Reported Date by clicking on the Date hyperlink

 

 

 

In “Maintain SA Incident screen, Go to Incident Locations and add Location

 

 

Complete the mandatory fields, and click on “Find Address”

 

 

 

If the location is known and you have the postcode, enter this in the appropriate boxes. Add post code and find address. The premises number or name can be entered, if you have these details. Then click on Find Address.

 

 

 

 

 

Select the appropriate address from the list by clicking on the Address Identifier hyperlink. When an individual address is displayed, you will need to select it by clicking the hyperlink.

 

If the location is unknown, add % plus known in “Premises Name” box. Click on Find Address and select “Address Unknown” from the table.

 

 

 

 

Click on “Save” icon

SG04 - Incident Location address has not been entered

Error Colour Code:

Error Brief Description: Error populates if the location address is left blank.                      
Location: Safeguarding Adults >SA incidents >Reported Date> Maintain SA Incident

How to fix this error?

 

 

 

Click on the hyperlink on the reported date

 

Tick View Details box

 

 

In Incident Location Detail, Click on Find Address, this will take you to the Address Search screen.

 

 

 

 

If you have the postcode, enter this in the appropriate boxes. The premises number or name can be entered, if you have these details. Then click on Find Address.

 

 

Select the appropriate address from the list by clicking on the Address Identifier hyperlink. When an individual address is displayed, you will need to select it by clicking the hyperlink.

SG05 - Victim Details with no "Resulting From"

Error Colour Code:

Error Brief Description:  Error populates if one of the options is not selected. 
Location: Safeguarding Adults>SA Incident>Maintain SA Incident> Victim Details

How to fix this error?

 

Go to Safeguarding Adult tab in the purple side bar menu in AIS. Click on SA incidents tab.

 

 

Select the appropriate Reported Date by clicking on the Date hyperlink

 

 

In Maintain SA Incident screen, go to Victim Details section.  From the drop-down list, select the appropriate ‘Safeguarding Concern’ option.

 

 

Click on “Save” icon

SG06 - Victim Details with no Severity

 

Error Colour Code:

Error Brief Description: Error populates if the severity is left blank in victim details in maintain SA incident screen

Location: Safeguarding Adults >SA incidents >Victim Details.

How to fix this error?

 

Go to Safeguarding Adults tab in the purple side bar menu in AIS. Click on SA Incidents tab. 

 

 

Click on the hyperlink on the reported date

 

 

In Maintain SA Incident screen, go to Victim Details section and select the appropriate option from the drop-down list.

SG07 - Missing "Abuse Start Date" in Victim Abuse Category field

Error Colour Code:

Error Brief Description: Error populates if the start date is left blank in Victim Abuse Category section in maintain SA incident screen. 

                  
Location: Safeguarding Adults >SA incidents >Appropriate reported date>Victim Abuse Categories

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Incidents tab 

 

 

 Select the appropriate Reported Date by clicking on the Date hyperlink

 

In Maintain SA Incident screen, Scroll down to “Victim Abuse Categories” section and enter the Abuse Start Date

 

Click on “Save” icon

SG09 ­- Client with no alleged perpetrator recorded

Error Colour Code:

Error Brief Description: Error populates if alleged perpetrator (individual or organisation) known to person at risk has not been added. 

                             
Location: Safeguarding Adults >SA Incidents > Alleged Perpetrators Linked to person at risk                        

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Incidents tab 

 

 Select the appropriate Reported Date by clicking on the Date hyperlink   

 

 

In Maintain SA Incident screen, go to Alleged Perpetrators Linked to person section.  Click on Add Perpetrator

 

 

 

Select Find Person or Find Org. as appropriate.  This will take you to the Person search or organisation search screen. 

 

 

In the Person Search Screen, enter the Forename and Surname of the perpetrator.

Click on “Find Person”

 

If you are searching for an Organisation, enter the name of the Organisation in the appropriate box.

 

 

Select the appropriate person from the checkbox

 

Go to Involvement tab in the purple sidebar menu in AIS. Click on Involvement

 

 

Click on Add Inv.

 

 

Click on Find Person or Find Org. accordingly and search for the perpetrator

 

Select the appropriate person or organisation by clicking on the identifier.

 

 

Scroll down in Maintain Involvements Screen to Add Role

 

From the drop-down menu select Alleged Perpetrator or Alleged Perpetrating Organisation accordingly

 

 

Click on “Save” button.

 

 

You can now see the Alleged Perpetrator’s name in Browse Involvement screen

SG10 - Missing "Person has Capacity"

Error Colour Code:

Error Brief Description: Error Populates if in Enquiry Screen, the radio button remains as "unknown" for the question "Is a mental capacity assessment required? Radio button defaults as unknown and error triggers if it is not changed to Yes or No.


Location: Safeguarding Adults>SA Summary>Enquiry

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Summery tab 

 

 

Click on the Name hyperlink on Enquiry section

 

 

 

In the Subjects section of the Enquiry screen, the radio button to question “person has capacity” is defaulted as unknown.  The radio button cannot remain as unknown and must change to “Yes” or “No” accoringly.

SG11- Enquiry Screen with no "Risk Level" recorded

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Summary tab 

 

 

Click on the Name hyperlink on Enquiry section

 

In the Subjects section of the Enquiry screen, select the appropriate risk level from the drop-down list.

Error Colour Code:

Error Brief Description: Error Populates if there is an investigation but not linked to the incident.
Location: Safeguarding Adults>SA Summary>Hyperlink for Strategy Discussion> Incidents> Link existing incidents

 

How to fix this error?

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Summary tab 

 

Click on the name hyperlink in Investigation

 

 

Go to Incidents section and click on “Link Incidents”

 


Select any incidents relevant to the investigation by ticking the relevant boxes in the select column, then click OK.

 

 

Click on “Save” button

SG12A - Open Enquiry with no link to Incident

Error Colour Code:

Error Brief Description: Error populates if there is an open enquiry, but no incident link to it.
Location: Safeguarding Adults>SA Summary>Enquiry> Incidents> Link existing incidents

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Summary tab 

 

 

Click on the name hyperlink in Enquiry

Go to Incidents section in the Enquiry screen, and click on “Link Incidents”.

 

Select the appropriate incident by ticking the relevant boxes in the select column, and click” OK”

 

Click on “Save” icon

SG14 - Episode ended at Investigation, but linked incident still open

Error Colour Code:

Error Brief Description: Error populates if the episode ends at investigation but the linked incident in investigation is still open.
Location: Safeguarding Adults>SA Summary>Investigation> Incidents> Reported date>Incident Details

 

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Summary tab

 

 

 

Open Investigation screen by clicking on the Name hyperlink

 

 

Go to Incidents section, and click on the Date hyperlink.

 

 

In Incidents Details, enter the End Date and SAVE.

SG14A - Episode ended at "Enquiry" but linked incident still open

Error Colour Code:

Error Brief Description: Error populates if the linked incident is still open. It triggers if the Enquiry has ended but the linked incident is still open.
Location: Safeguarding Adults>SA Incident> Browse SA Incident> Reported date> Incident Details

How to fix this error?

 

If the  “Enquiry” has ended, the incident must also be ended, otherwise it populates error.

 

 

All sections become greyed out  when episode ends in Enquiry, , which means no alteration is attainable.   In order for any alteration, the episode requires to be open again.

For opening the Enquiry, scroll down to completion section in Enquiry screen, and then change the radio button for “Is the enquiry complete?” from “Yes” to “No” and SAVE.

 

 

Go to SA Incidents  in Safeguarding Adults, and click on SA incidents tab.

 

 

 

In Browse SA Incidents, click on the appropriate Reported Date hyperlink

 

 

In Maintain SA Incident screen, scroll down to Incident Details. Enter the relevant End Date and SAVE.

 

 

After the amendment is completed, we need to end the episode again.

 

Click on SA Summary, and click on name hyperlink in Enquiry

 

 

In Enquiry screen,scroll down to completion section, and change the radio button for “Is the enquiry complete?” from “No” to “Yes”.

 

Click on “Save” icon

SG15 - Episode ended at Case Conference but linked investigation still open

Error Colour Code:

Error Brief Description: Error populates if the episode has ended at conference but the linked incident in conference is still open.
Location: Safeguarding Adults>SA Summary>Case Conference> Incidents> Reported date>Incident Details

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Summary tab

 

Open Conference screen by clicking on the Name hyperlink

 

Go to Incidents section in Conference screen, and click on the Date hyperlink.

 

In Incidents Details, enter the End Date and SAVE.

Error Colour Code:

Error Brief Description: Error populates if the incident has ended but linked Enquiry is still open.
It triggers if the incident has ended but the Enquiry has no end date recorded.
Location: Safeguarding Adults>SA Summary >Enquiry>Completion

 

How to fix this error?

 

If the  “Incident” has ended, the Enquiry must also be ended.

 

Go to SA Summary in Safeguarding Adults, and click on SA Summary tab.

 

Click on name hyperlink in Enquiry

 

 

In Enquiry screen, scroll down to completion section, and enter the relevant Enquiry End Date. “Is the enquiry complete”? Click on “Yes” radio button.

 

Click on “Save” icon

SG16 - Missing Category of Relationship

Error Colour Code:

Error Brief Description: Error Populates if “Category of Relationship” has not been selected from the drop down menu in maintain SA incident screen.  This is located in "Alleged Perpetrators linked to Victim" section, and it populates error if it is left blank.


Location: Safeguarding Adults > SA Incidents >Maintain SA incident> Alleged Perpetrators Linked to Victim

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Incidents tab 

 

 

Select the appropriate Reported Date by clicking on the Date hyperlink

 

 

Go to Alleged Perpetrators Linked to victim section and tick the View Details checkbox

 

 

Select the appropriate Category of Relationship from the drop-down list

Error Colour Code:

Error Brief Description: Error Populates if question "Who is the Responsible CSSR" has not been selected including Out Of County cases.                              

Location: Contacts and Referrals > Click on appropriate date > Details

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on Contacts tab

 

 

Select the appropriate Reported Date by clicking on the Date hyperlink. This will take you to Maintain Contact screen

 

 

 

 

 

In Maintain Contact screen, go to Details section. To add “Who is the Responsible CSSR”, click on the Find Organisation button and enter in the name of the Local

Authority that is responsible for the Adult at Risk. Click on the “Identifier” to select the Local Authority. 

 

 

Click on the “Identifier” to select the Local Authority.

Go to Contacts and Referrals tab in the purple sidebar menu in AIS. Click on Contacts tab

 

 

Select the appropriate Date by clicking on the Date hyperlink

 

 

In Main Contact screen, scroll down to Details Section and select the appropriate

Outcome from the drop-down menu.

Error Brief Description: Check to ensure that the date of the outcome is not left empty.  Error populates if the outcome date has not been entered within 48 hours of the contact date.

How to fix this error?

 

Go to Contacts and Referrals tab in the purple sidebar menu in AIS. Click on Contacts tab 

 

 

Select the appropriate Contact by clicking on the Date hyperlink

 

 

 

In Main Contact screen, scroll down to Details section Enter Outcome Date

How to fix this error?

 

Go to Assessment tab in the purple sidebar menu in AIS. Click on Assessments

In Maintain Assessment screen and Assessment Details, enter the Actual Start Date

How to fix this error?

 

Go to Assessment tab in the purple sidebar menu in AIS. Click on Assessments

 

Click on appropriate “Safeguarding Enquiry Assessment” hyperlink.

 

 

In Maintain Assessment screen and Assessment Details, enter the End Reason by selecting the appropriate option from the drop-down list.

Error Colour Code:

Error Brief Description: Error populates if at least one from the four Personal Outcome/Goals is not selected within Safeguarding Enquiry Assessment questions.                                                               
Location: Assessment >Safeguarding Enquiry Assessment>Maintain Assessment >Click on Navigation tab> Desired Outcomes/Goals of the Adult At Risk/Representative

How to fix this error?

Go to Assessment tab in the purple sidebar menu in AIS. Click on Assessments

 

 

Click on appropriate “Safeguarding Enquiry Assessment” hyperlink

 

 

In Maintain Assessment screen, click on the yellow “Navigation” menu at the top right hand corner

 

Click on the “Desired Outcomes/Goals of the Adults at Risk” question.

 

 

There are 4 sections which consist of a text box which can be used to type freestyle the Adult at Risk’s desired outcome/Goals. Select the relevant status from the drop-down menu.

 

Click on “Save” icon

SG32 - Safeguarding Enquiry Assessment with missing "Lead Organisation"

Error Colour Code:

Error Brief Description: Error populates if there is no option selected for the” lead organisation” question in Safeguarding Enquiry section within Safeguarding Enquiry Assessment.  It triggers if it is left blank.                                           
Location: Assessment>Safeguarding Enquiry Assessment>Maintain Assessment>Click on Navigation tab>

How to fix this error?

 

Go to Assessment tab in the purple sidebar menu in AIS. Click on Assessments

 

 

Click on appropriate “Safeguarding Enquiry Assessment” hyperlink

 

In Maintain Assessment screen, click on the yellow “Navigation” menu at the top right hand corner

 

 

 

Click on the “Safeguarding Enquiry” section, and select the appropriate Lead Organisation from the drop down list in the question. There is a text box which can be used to type freestyle.

 

 

Click on “Save” icon

Error Brief Description: Error Populates if the responsible person has not been identified in maintain assessment screen, assessment details section.
Location: Assessments>Safeguarding Enquiry Assessment >Maintain Assessment screen

How to fix this error?

 

Go to Assessment tab in the purple sidebar menu in AIS. Click on Assessments

 

 

Click on appropriate “Safeguarding Enquiry Assessment” hyperlink

Go to Assessment tab in the purple sidebar menu in AIS. Click on Assessments

 

 

Click on appropriate “Safeguarding Enquiry Assessment” hyperlink.

 

 

Once the assessment has been authorised by the relevant manager, in Maintain Assessment screen and Assessment Details enter the date “Given to Client” and if relevant “Date Given to Carer”

Error Brief Description: Error Populates if an episode has ended, but no Safeguarding survey contact recorded at the end of the episode. It triggers if any episode has ended since 01/11/2017, but no survey contact recorded.

How to fix this error?

 

Once the episode has been completed, all victims should be offered the opportunity to complete a survey based on how they felt their safeguarding concern was dealt with. This offer can be accepted or declined but we still need to record this for reporting purposes.

 

Go to Contacts and Referrals tab in the purple sidebar menu in AIS. Click on Contacts

 

 

 

In Browse Contacts screen, click on Add Contact

 

 

Add in the date the victim was contacted and offered the opportunity to complete the survey.  Record the time the subject was contacted.

 

 

Why is the contact being made?”  Always Safeguarding Survey

 

 

 

 “What was the outcome of the contact?” – From the drop down menu, select    Safeguarding Survey Accepted or Safeguarding Survey Declined according to the victim’s decision.

Error Colour Code:

Error Brief Description: If the Enquiry has ended at the Stage 1 Enquiry, then the "Status" in SA incident needs to be changed to "INCIDENT CEASED".  If unchanged, it triggers error.

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Summary tab

 

 

Go to Investigation sceen by clicking on the name hyperlink

 

 

In the Investigation screen within SA Summary you would choose the Decision of “Concluded in Stage 1.

If the “Investigation Decision” indicates “Concluded in Stage 1”, then the relevant SA incident needs to be updated accordingly and change to “INCIDENT CEASED”.

 

 

Click on “Save” icon

SG51A- Episode concluded at Incident but incorrect "Status" on SA incident

Error Colour Code:

Error Brief Description: If the episode has ended on Incident and no Enquiry is needed to be completed, then the "Status" in SA incident needs to be changed from Alleged Pending Enquiry or Suspect Pending Enquiry.   If unchanged, it triggers error.

How to fix this error?

 

Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click on SA Incidents tab

 

 

Select the appropriate Reported Date by clicking on the Date hyperlink

 

 

In Maintain SA Incident screen, go to Incident Details section. Add the date the incident is closed. From the drop- down menu, change the status from Alleged-Pending Enquiry" OR "Suspected- Pending Enquiry" to an appropriate option. At this stage it is normally ‘Incident Ceased’.

 

 

Add the date the abuse category ended

 

 

 

 

Open up the alleged perpetrator record to finish off closing the incident.

 

 

Add outcome and select an appropriate outcome from prefixed ‘1’.

As above, if the outcome selected is No Risk Identified and No Action Taken; there is no need to choose from ‘2 –‘prefixed outcome.

 

If a ‘1 – Risk Identified’ prefixed outcome is chosen, then a ‘2 –‘prefixed outcome must also be added by clicking on the ‘Add Outcome’ button

 

 

Click on “Save” icon

If the Victim Abuse Category has been selected as “Self-Neglect”, then the Category of Relationship must be selected as “OTHER KNOWN”

 

 

Go to Alleged Perpetrators Linked to victim section and tick the View Details checkbox. Select “Other Known” for Category of Relationship.
