go to Incident Details section; select the appropriate contact with Safeguarding
Concern from the drop- down list.
Click on

"Status"

Error Brief Description: Error populates if an incident has ended but a status of
“Alleged-Pending Enquiry or "Suspected -Pending Enquiry" has remained
unchanged. Need to be changed to one of the other options from the drop down
list.

How to fix this error?
Go to Safeguarding Adult tab in the purple side bar menu in AIS. Click
on SA incidents tab.
Select
the appropriate Reported Date by clicking on the Date hyperlink
In “Maintain SA Incident screen, Go to Incident Locations and add
Location
Complete
the mandatory fields, and click on “Find Address”
If the
location is known and you have the postcode, enter this in the appropriate
boxes. Add post code and find address. The premises number or name can be
entered, if you have these details. Then click on Find Address.
Select
the appropriate address from the list by clicking on the Address Identifier
hyperlink. When an individual address is displayed, you will need to select it
by clicking the hyperlink.
If the
location is unknown, add % plus known in “Premises Name” box. Click on Find
Address and select “Address Unknown” from the table.
Click on
“Save” icon

Select the appropriate Reported Date
by clicking on the Date hyperlink
In
Maintain SA Incident screen, go to Alleged Perpetrators Linked to person
section. Click on Add Perpetrator
Select
Find Person or Find Org. as appropriate.
This will take you to the Person search or organisation search
screen.
In the
Person Search Screen, enter the Forename and Surname of the perpetrator.
Click on
“Find Person”
If you
are searching for an Organisation, enter the name of the Organisation in the
appropriate box.
Select
the appropriate person from the checkbox
Go to Involvement
tab in the purple sidebar menu in AIS. Click on Involvement
Click on Add
Inv.
Click on Find
Person or Find Org. accordingly and search for the perpetrator
Select the appropriate person or
organisation by clicking on the identifier.
Scroll
down in Maintain Involvements Screen to Add Role
From the
drop-down menu select Alleged Perpetrator or Alleged Perpetrating Organisation
accordingly
Click on
“Save” button.
You can
now see the Alleged Perpetrator’s name in Browse Involvement screen

is still open. It triggers if the Enquiry has ended but the linked incident is
still open.

Reported date> Incident Details
How to fix this error?
If
the “Enquiry” has ended, the incident
must also be ended, otherwise it populates error.
All
sections become greyed out when episode
ends in Enquiry, , which means no alteration is attainable. In order for any alteration, the episode
requires to be open again.
For
opening the Enquiry, scroll down to completion section in Enquiry screen, and
then change the radio button for “Is the enquiry complete?” from “Yes” to “No”
and SAVE.
Go
to SA Incidents in Safeguarding Adults,
and click on SA incidents tab.
In
Browse SA Incidents, click on the appropriate Reported Date hyperlink
In Maintain SA Incident screen, scroll down to
Incident Details. Enter the relevant End Date and SAVE.
After the amendment is completed, we need to end
the episode again.
Click on SA Summary, and click on name hyperlink in
Enquiry
In
Enquiry screen,scroll down to completion section, and change the radio button
for “Is the enquiry complete?” from “No” to “Yes”.
Click on “Save” icon

Description: Error
Populates if an episode has ended, but no Safeguarding survey contact recorded
at the end of the episode. It triggers if any episode has ended since
01/11/2017, but no survey contact recorded.

How to fix this error?
Once
the episode has been completed, all victims should be offered the opportunity
to complete a survey based on how they felt their safeguarding concern was
dealt with. This offer can be accepted or declined but we still need to record
this for reporting purposes.
Go to Contacts and Referrals tab in the purple sidebar menu in AIS.
Click on Contacts
In Browse Contacts screen,
click on Add Contact
Add
in the date the victim was contacted and offered the opportunity to complete
the survey. Record the time the subject was
contacted.
“Why is the contact being made?” Always Safeguarding Survey

to the victim’s decision.

CEASED". If unchanged, it triggers
error.

In the Investigation
screen within SA Summary you would choose the Decision of “Concluded in Stage
1.
If the “Investigation
Decision” indicates “Concluded in Stage 1”, then the relevant SA incident needs
to be updated accordingly and change to “INCIDENT CEASED”.

SG51A- Episode concluded at Incident but incorrect "Status" on SA
incident

Description: If the episode has ended on
Incident and no Enquiry is needed to be completed, then the "Status"
in SA incident needs to be changed from Alleged Pending Enquiry or Suspect
Pending Enquiry. If unchanged, it
triggers error.

How to fix this error?
Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click
on SA Incidents tab
Select the appropriate Reported Date by clicking on the Date hyperlink
In
Maintain SA Incident screen, go to Incident Details section. Add the date the
incident is closed. From the drop- down menu, change the status from Alleged-Pending Enquiry" OR "Suspected- Pending
Enquiry" to an appropriate option. At this stage it is normally ‘Incident Ceased’.
Add the
date the abuse category ended
Open up
the alleged perpetrator record to finish off closing the incident.
Add outcome and select an appropriate outcome from prefixed
‘1’.
As above,
if the outcome selected is No Risk
Identified and No Action Taken; there is no need to choose from ‘2
–‘prefixed outcome.
If a
‘1 – Risk Identified’
prefixed outcome is chosen, then a ‘2 –‘prefixed
outcome must also be added by
clicking on the ‘Add Outcome’ button

“Save” icon

if the Enquiry indicates ‘Yes’ for completion.

How to fix this error?
Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click
on SA Summary tab
Click on the Name hyperlink on
Enquiry section
In
Enquiry screen, scroll down to Outcomes
section, and click on Add Outcome
Select
the appropriate outcome (mandatory) from the drop-down list.
Add Outcome Date (mandatory), and select
the appropriate option from the drop-down list.
Click on
“Save” icon

SG54 - Case Conference recorded with missing "Outcome"

Error Brief
Description: Error populates when the recent Case Conference has
missing outcome. It triggers if the conference indicates Yes
for completion.

How to fix this error?
Please
note you will need to change the answer to “is the Conference complete?” to
“No” in Case Conference in order to make amendments, and then change it back to
complete once changes are made.

and the Category of Relationship is other than OTHER KNOWN to individuals.

How to fix this error?
Go to Safeguarding Adults tab in the purple sidebar menu in AIS. Click
on SA Incidents tab
Select
the appropriate Reported Date by clicking on the Date hyperlink
If the
Victim Abuse Category has been selected as “Self-Neglect”, then the Category of
Relationship must be selected as “OTHER KNOWN”
Go to
Alleged Perpetrators Linked to victim section and tick the View Details
checkbox. Select “Other Known” for Category of Relationship.
Click on
“Save” icon
